Name Section: Proper name should match the name shown on your photo ID. AKA (also known as) names should be names you CURRENTLY use when filling out loan applications and/or official financial documents (not nicknames, maiden names, etc.)
This Address Section should match what you will submit as your current physical proof of address. Other addresses (if any) should show another address you are currently using and okay getting mail at, including pre-approvals. (ie PO Box, Relative Home)
